Multiple options need for contact form

This is for an insurance companies website. They have a page called "easy quote". The user enters their standard personal info then they check the box(s) stating which type of quote they want. Then based on the options check the user then sees those forms on the screen for them to fill out.

This is tricky and I'm not sure how to do it. I'm pretty good with php but the client wants it all to happen on one screen so they fill evrything out at once and hit enter once. Then the results are emailed to him.

Any ideas?

For example.

If the user checks Auto, Home and Life those questions will be asked. And if the user checks health then only those questions will be asked.
