Looking for a program for a web questionnaire

I've tried various programs including Articulate Quizzmaker and others, but they do not offer exactly what I want. I am trying to design questionnaires that have Questions that will lead to different ses of questions and eventually an answer, but there are many possible answers, the one one would get should be determined by the answers to their questions. It is basically meant to be way to choose something based on answers to questions in my questionaire. Does anyone know of a program that could create such a questionnair (preferably in the formof a web form). Articulate Quizzes (the program that I've tried) seems to only b e for like online tests.

Wish I knew of an automated service, but it is something that could be done in php.

Have you looked at MasterQuiz?

It sounds like it may be a good fit. It can create simple one page quizzes and complex multiple level quizzes such as you describe.

There are working examples of both a single page and a multi-level quiz for you to take a look at, and a control panel demo.
