Feedback application

Can you recommend some simple application for collecting feedback from website visitors? I would like to put it in my website and get visitors opinions on how my website works. I can not find anything interesting yet .

Adding a contact form always helps. If you're using CMS software, add an option for the article to be rated and an option for users to comment on it.

provide a poll or something that rates their overall experience! and also one that ask which areas that "you choose" are the best parts of the site.

I think that you need guest book, but with strong captcha protection.
