I've got a form on my site that is fairly long and complicated. Not your standard 10 field, name, address, etc. I currently use a SurveyGizmo form on my site because it offers a whole bunch of different question types, but I'm looking to have someone create a form for me for a 1 time fee so I won't have to keep paying a monthly fee and so I can finally get a form that's *exactly* the way I want it without having to find work-arounds that are within the limitations of an off-the-shelf product like SurveyGizmo.
There's really not much design involved, but as I said, the form is full of if/then/else statements, show/hide commands, disqualifications, and re-routs. The form is too complex for me to write myself, so I'm wondering if anyone knows where I should go to find someone to create this web form for me. I also really have no idea how much something like this will cost, so I'm looking to see if anyone can give me a ballpark.
