PHP Questionnaire Form

I am trying to create a questionnaire form using php for my website. I use the sitewizards php script create to my contact forms. Basically I want to alter the script (feedback.php) to work with my questionnaire form. I only want name, company, telephone, email, comments to be required.

Attached is the feedback.php script created by site wizard and the html form I have made for the questionnaire.
