Forms script

can anyone recommend a good script for an enquiry form?

I made one on this page utilising my hosts standard form hander script, the page is www.aptb82.dsl.pipex.com/kevinsutton/Contact2.htm but the confirm page does not work, does anyone know why this is?

There are lots of CGi based form processing options listed here

I have used the BNB one in the past and it works well.
