does anybody help me to get the contact us form for my website...the form will have these fields..Name: Contact persons email : message and Mobile number...replies appreiated...thanks

This is developed for MasterFormV4 (http://dlperry.com/masterform.html) and MasterFeedback (http://dlperry.com/masterfeedback.html), but it can also work for other form handling applications.
