I'm building a web site for a dealership that I work for. Our sales people are assigned different sections of the state (note: the structure of this assignment is done by county)

We would like to have a script or a way that a customer types in his/her zip code (or county) and then they are able to contact that salesperson directly by an html form.

Is there a script that allows for this or does anyone have any suggestions? I thought about having a drop down menu with a list of all counties and then finding a form that will send the email depending on which county is selected. I know I've seen this type of form in the past, I just don't know where. Any ideas or advice would be greatly appreciated!
