Reusable "Email link to this page" form

After a user takes an action such as signing up for a mailing list, I like to present the user with a form to invite their friends to the website via Email. I have a form that I generally use, but I am looking for a better one (see levees.org/thankyou?for=7).

Does anyone know of a reusable, preferably AJAX, form that I can download and implement onto all of my clients' sites?

If not, is there much interest in such a form if I were to set out to code one for the community?
