Default Interactive Form

I'm stumped on this one. I am creating a website for a local baseball school and want to create a form to sign up for lessons. It will basically be a table displaying all possible times, with days at the top, and times along the side. Customers will be able to go to the page, click in the desired cell for day and time, and simply type in their name to sign up. I have a small amount of knowledge on forms, so far everything I make is automatically erased when the page is refreshed. To further complicate things, I have a limited amount of width to insert this form or table so I need something with scrollbars.
