Default FP 2003 Form Requires UN & PW

I been using Frontpage 2003 to manage/design my Alumni Website I created at www.mcclainhs.com . I've had it up over a year now.

I have been adding alumni profiles using FP2003 Form located on this page here: www.mcclainhs.com/profile.htm . I have NEVER had an issue with this form as it has worked perfectly until recent. I attempted to apply a UPLOAD component (for profile pictures) to the form I been using and decided that it was no need for it right away so removed it. I done nothing different in the form properties at all. (none/zero/ziltch) But I saved the page prior to closing it although I done nothing to change it. (Out of habit actually)

I did however try a test submission to make sure it was working (hadnt received profile in a few days is why) and when I hit submit a dialog box appeared prompting me to enter a UN & PW. Here is the content of the dialog box:

The server mcclainhs.com at : requires a username and password.

Warning: This server is requesting that your username and password be sent in an insecure manner (basic authentication without a secure connection).


The folder the info is sent to (server side) is able to be written to (chmod), and I also have a copy forwarded to my email account as well. I have uninstalled, reinstalled FP Extensions multiple times now with no success.

What is funny though is that this form will work from time to time so I'm not always getting the pop-up box requesting my UN & PW.

Any ideas would be helpfull. I have searched high and low on this topic (google/yahoo/microsoft) and have found others w/ the same issue, with NO results.
